A network of community clinics struggled with data fragmentation. We built 'CareNet', a centralized health information exchange (HIE) portal. We implemented the HL7 FHIR standard to ensure the platform could communicate with various existing clinic software. The portal utilizes a secure, multi-tenant cloud architecture that provides doctors with a 360-degree view of patient history, including medications, allergies, and past visits. We also included a patient-facing module, allowing individuals to book appointments and view their results from any device, enhancing engagement and health literacy.

Phase 3: Secure Identity Management setup including multi-clinic authentication. Phase 4: Portal Development focusing on speed and readability for medical staff. Phase 5: Data Migration & Cleansing for over 500,000 legacy patient records. Phase 6: On-site Training & Pilot Launch in three high-volume community centers. The implementation successfully transformed the client's operations, setting a new benchmark for efficiency and reliability in their sector. By combining cutting-edge technology with a user-centric design approach, we delivered a platform that scales with their ambition.
