01. The Challenge

A network of community clinics struggled with data fragmentation. Patient records were often stored in local silos, making it impossible for a doctor at one clinic to see the history should the patient visit another center. This led to redundant tests, delayed care, and higher costs. They needed a secure, interoperable portal that could act as a single source of truth for patient history, while being simple enough for staff in resource-constrained environments to use efficiently.

02. Our Solution

We built 'CareNet', a centralized health information exchange (HIE) portal. We implemented the HL7 FHIR standard to ensure the platform could communicate with various existing clinic software. The portal utilizes a secure, multi-tenant cloud architecture that provides doctors with a 360-degree view of patient history, including medications, allergies, and past visits. We also included a patient-facing module, allowing individuals to book appointments and view their results from any device, enhancing engagement and health literacy.

Patient Longitudinal Record: A unified chronological view of all medical interactions across the alliance.
Secure Portal for All: Role-based access for doctors, nurses, admin staff, and patients.

Medication Reconciliation: Automated checks for potentially dangerous drug-to-drug interactions.
Real-Time Lab Results: Instant notification for both providers and patients when results are ready.

03. The Process
